Functional EMR Design: A&P Problem Selection and Coding
Refreshing the infrastructure of the Assessment and Plan
In this continuation of the Functional EMR Design series, we see how our fictional “oriole” EMR handles the basics of the Assessment and Plan process, including problem selection and coding. It continues to address one of the biggest functional disconnects in our clinical systems: the constant friction between the medical problems clinicians treat, and the coded diagnoses used for billing purposes.
This video follows on the trail of the previous episode, Diagnosis Search and Select, both of which make reference to this article: The “Coding For” Fallacy. Worth a look if you haven’t seen them.
If you are an EMR user, designer or enthusiast, especially in primary care, this video may interest you. It’s time to challenge the industry to make change. We’ll never see real improvements, or realize the benefit of AI in primary care, until we refresh the infrastructure, piece by piece.

This is great! The fracturing of the story across different diagnoses is a true problem today.
Oriole EMR is wonderful...
